Safe details for a first enquiry
Share your name, contact method, the suburb and the tasks you want help with. A participant’s first name or initials are enough for a referral.
Leave sensitive records out
Do not email a diagnosis, NDIS number, medical records, medication list or a copy of the plan.
If someone contacts us for you
Tell them what they may share, who should be contacted and how you want to take part. They should tell Share the Care that they have permission to share the details.
Before sending more information
- Ask why each item is needed
- Ask who will see it and how it will be used
- Agree a safer way to send any documents
- Keep a copy of what you provide
Ask about information already held
Contact Share the Care if you want to ask what information is held about you, request a correction or raise a privacy concern. Ask for the applicable privacy policy and process.
